The Process of Transition

We ensure students are prepared for their journey into adulthood.

OUR APPROACH

At the age of fourteen, our school social worker begins the process of transition for each student. This process includes:

    • IEP Goal Development for Daily Living Activities, Job Exploration and Community Integration
    • Resources for Estate Planning & Administration
    • Turning 18: Guardianship, Social Security and Waiting Lists
    • Medicaid, SSI & Other Important Entitlements
    • Transition From School to Adulthood & Graduation Considerations
    • Resources for Wills & Trusts

Our school works collaboratively with local agencies to plan and develop a timeline for the transition after the student graduates from our school at the age of twenty-one. Various parent workshops, training, and meetings are held to ensure this timeline is met, and students are prepared for their journey into adulthood.
